Abstract:

Purpose: To evaluate the effects of laser therapy in patients with CHOROIDAL neovasculatization refering to Labbafinejad medical center during 1994-99. Materials and methods: This descriptive study was based on existing data. By review of patients records, the following information were extracted: age, sex, job, corrected visual acuity before and after laser therapy (in the last follow up), the status of fundus appearance, some of known risk factors, and angiographic findings. The patients were divided into 3 groups; subfoveal, extrafoveal, and juxtafoveal based on location of CHOROIDAL NEOVASCULARIZATION. Statistical analysis of data performed using computer data base. Results: Most of the patients were male, mean follow up was 9.25 months (9.25± 12.97), mean age of patients was 55/6 (14-79) years. Overall in all 3 groups after macular photocoagulation, final visual acuity increased, but the difference was not statistically significant(p< 0.29). The mean visual acuity increased from 198/1000 to 257/1000 after a mean follow up of 9.25 months. In 9 eyes (34.6%) with subfoveal involvement the mean visual acuity raised trom 53/1000 to 57/1000 during a mean follow up of 14 (14±19) months (p< 0.54), in 10 eyes (38.4%) with extrafoveal involvement the mean visual acuity raised from 373/1000 to 402/1000 after a mean follow up 7 (7±6.4) months (p< 0.70), and in 7 eyes (27%) with juxtafoveal involvement, the mean visual acuity raised from 137/1000 to 310/1000 during a mean follow up of 6 (6±5.9) months (p< 0.37). The most frequent risk factor was age-related macular degeneration. Conclusion: Although the results of macular photocoagulation for CHOROIDAL NEOVASCULARIZATION are still disappointing, it may prevent further decrease in visual acuity in some selected patients.

Abstract:

Purpose: To report a case of CHOROIDAL osteoma associated with reactivation of CHOROIDAL NEOVASCULARIZATION (CNV) and development of focal CHOROIDAL excavation (FCE). Case Report: A 34-year-old woman with CHOROIDAL osteoma complicated by CNV in the right eye for two years presented with deterioration of visual acuity in her right eye. A small retinal hemorrhage accompanied by subretinal fluid (SRF) was seen in the macular area of the right eye. Optical coherence tomography (OCT) showed that the inner retina was intact, and the outer retinal layers had outward displacement. SRF and a wedgeshaped CHOROIDAL depression were also seen. This CHOROIDAL excavation was not present on previous OCT images. The integrity of the inner retinal layers was maintained, and an optically clear space was present between the neurosensory retina and the retinal pigment epithelium. Conclusion: CHOROIDAL osteoma can be complicated by CNV and FCE could occur as a consequence. Again, FCE can lead to CNV development. This cascade can deteriorate vision and sometime lead to permanent visual loss.

Abstract:

To report a patient with juxtapapillary CHOROIDAL osteoma (CO) with serous retinal detachment (SRD) not associated with CHOROIDAL NEOVASCULARIZATION (CNV) who was successfully treated with a single intravitreal injection of bevacizumab. A 20‑year‑old woman presented with decreased vision in her right eye. Ultrasonography, fundus autofluorescence, fluorescein angiography (FA) and optical coherence tomography (OCT) were performed. She was diagnosed with juxtapapillary CO with SRD in the absence of CNV. The treatment involved a single intravitreal injection of bevacizumab. Visual acuity (VA) was 20/100 pre-injection which was dramatically improved to 20/20 one week after the injection without any additional treatment. Fundus examination and OCT one week after the injection revealed complete absorption of the subretinal fluid (SRF) in the macula. Eighteen months after the injection, vision remained stable and she did not show re‑accumulation of SRF in her right eye. Intravitreal injection of bevacizumab may be effective in managing CO associated with SRF without CNV.

Abstract:

Purpose: To report the closure of a recalcitrant macular hole (MH) following the development of CHOROIDAL NEOVASCULARIZATION. Methods: A 67‑, year‑, old female patient in this case report was diagnosed with a MH and operated twice, but anatomical closure of MH could not be achieved. The patient was followed up without further treatment, as she rejected any additional procedure. Results: Six months later, a lesion consistent with CHOROIDAL NEOVASCULARIZATION appeared in the central macula, and the recalcitrant MH closed spontaneously. The MH defect remained closed in the following years. Conclusion: Besides being a new example of the presence of CHOROIDAL NEOVASCULARIZATION after MH surgery, the most important aspect of this case report is to report the closure of a recalcitrant MH following the development of CHOROIDAL NEOVASCULARIZATION.

Abstract:

Purpose: To Report a case of bilateral CHOROIDAL NEOVASCULARIZATION (CNV) and angioid streaks in a patient with  thalassemia. Case Report: A 41-year-old male, a known case of -thalassemia, presented with the gradual decrease of vision from 3 months ago. The best corrected visual acuity was 8/10 in right and 3/10 in the left eye and Marcus Gunn was negative. In slit lamp examination, the anterior segment of both eyes was normal and in the posterior segment of both eyes, dark brown to red irregular bands radiating a few millimeters from a peripapillary ring was visible. There was an active CNV in the right eye and a CNV scar in the left eye. Intravitreal injection of bevacizumab was performed for the right eye. Conclusion: Angioid Streaks can occur in patients with 􀀀 thalassemia and one of the complications of angioid streaks is CHOROIDAL NEOVASCULARIZATION, which can decrease vision.

Abstract:

A 23-year-old girl presented with blurred vision in her left eye since 2 months ago. Her past medical history was unremarkable. On presentation, her left best corrected visual acuity was 20/80. Anterior segment exam was unremarkable. Posterior segment exam showed an ill-defined green gray elevation in the fovea with surrounding subretinal fluid. The right eye exam was normal. Fluorescein Angiography and Optic Coherence Tomography of the left eye was compatible with CHOROIDAL NEOVASCULARIZATION (CNV).
